Complete your Intake Form

• You will need to create an account from the home page. This is free of charge and will give you access to the Intake form and the Dashboard.
• The Dashboard is where all communication will occur between the veteran and K&DVMA staff. You will be able to upload your records, communicate with K&DVMA staff, pay for services, receive documents etc.
• The Intake form is a very simple form that K&DVMA uses to gather basic information in regards to your military service, previous & current medial information, and your contact details.
• The Intake form should be filled out immediately. Your prompt, complete and accurate completion of the Intake form will ensure a licensed physician and/or a case manager contacts you promptly to explain the process in more detail based on the information provided.

Preliminary Discussion

• This is when a senior staff member will contact you and discuss the information you provided on your intake form.
• It is HIGHLY recommended that you try to be in a location where you can discuss your past and current medical history.
• You will be informed on what documentation you will need to upload during the process to ensure an accurate review of your record is done.
• You will receive a price quote depending on the complexity of your case.
• Once your record has been thoroughly reviewed to identify any potential service related medical connection, a case manager will be assigned to assist you in your process from this point.

Pay your Fees Online

• You will receive an invoice via email or you may go to “pay your fees online” via your dashboard. Your fee is given to you during the “Preliminary Discussion"
• Fees will vary based on the complexity of the case.
• Payment is free and easy to use. Simply log in to your account through the KD VMA website to access the Dashboard where you will be able to make a secure payment using the Pay Pal service.

Upload Medical Record

• "Upload Medical Record" should take you to another page that explains this process in detail.
• You will need to gather any and all medical records from your time on active service and post active service if applicable. If you do not have them ensure you let us know during the “Preliminary Discussion”.
• Instructions on what documents to upload and how they should be labeled.
  • DD214 or NGB22 for National Guard veterans only.
  • Active duty medical record
  • Civilian medical records
  • VA medical records
  • Radiologist report on imaging(MRI, X-ray, CT, Ultrasound, etc..) DO NOT UPLOAD ACTUAL IMAGING!!!
  • VA rating letters if applicable.

Receive your IMO/DBQ directly from our Doctor

• “Receive your IMO/DBQ directly from our Doctor”
• After your records were thoroughly reviewed you will receive an Independent Medical Opinion (IMO) or Disability Benefits Questionnaire (DBQ) from one of our licensed physicians. This is one of the most important steps in our process.
• You will review your IMO’s or DBQs, and check for accuracy of information. Corrections will be made by our staff and you will approve the document before the final signature from the physician.
• No new conditions will be added to your case once all documents are signed by the physician. It is imperative you let our staff know during the “Preliminary Discussion” if you will be seeking compensation for a condition not listed on your intake form.
• Once you have obtained the final IMO or DBQ, you can start your claim process and be ready to start your VA Compensation & Pension Exam’s (C&P).

File your VA Claim

For veterans that have never filed, the VA has three programs for disability compensation. Which program you file under will be determined by where you are at in your transition at time of filing.
